BOOKING POLICY

I agree to have eyelash extensions applied to my natural eyelashes and/or removed

I understand that during this procedure it is my responsibility to keep my eyes closed and follow the direction of the certified lash technician to avoid any chemical burns until the certified eyelash technician addresses me to open my eyes.

I understand that on rare occasions there are risks associated with having artificial eyelashes applied to or removed from my natural eyelashes.

Furthermore, I understand that in rare cases as part of the procedure, eye irritation and discomfort can occur. 

​

I agree that if I experience any of these reactions I will contact the certified eyelash technician that performed the procedure and it may be beneficial to remove the eyelash extensions.

​

I agree to disclose any allergies or sensitivities I may have, to the technician prior to the service and during procure if it occurs.

​

I understand that I am required to follow the aftercare instructions provided by the certified eyelash technician in order to maintain the longevity of my eyelash extensions, and any touch-up service is at the discretion of the technician upon evaluation. e.g. melted lashes, picking, retention, etc.

Refill Services are for lash sets completed within 8-21 days anything past 21 days is considered a full set and will be charged accordingly.

I understand if there is any concern with excessive shedding within the first 24-48 of application to contact the eyelash technician for a consultation and/or a touch-up, should it occur a photo will be required for the technician's records, anything 48hrs after application would be considered a mini fill noted on the booking site.

I understand that by reading and electronically signing this form I am releasing the certified eyelash technician and HerGlory from any claims or damages of any nature.

​

I am aware there is an initial  25% deposit 

I understand that once a payment is made, services received are non-refundable unless stated on a case-by-case basis.

Testimonials and photos may be taken during your service and used as marketing/promotional content, to opt out please DM me on Instagram.
